Is it mandatory to upload PreAuthorisation Request Form (PRF)?

Yes, it is mandatory to print the PRF and get it signed by the Treating Doctor. The signed PRF has to be scanned and uploaded while initiating pre-authorization. The same would be verified during the claims stage.
